Résumé

RÉSUMÉ: L’insomnie empêche la satisfaction du besoin fondamental de dormir de quatre femmes sur dix âgées de 40 à 60 ans (Blümel et al., 2012). Fortement associée à l’insomnie, l’anxiété tend à augmenter tout au long de la transition vers la ménopause (Terauchi et al., 2010). L’insomnie et l’anxiété peuvent avoir des effets néfastes sur la santé physique et mentale, pourtant seulement 20 % des personnes qui se plaignent d’insomnie sont correctement prises en charge (Ohayon, 2007). L’intervention est un essai expérimental pré et post-intervention avec un groupe contrôle et un groupe expérimental ayant pour but d’évaluer l’efficacité d’une intervention en polarité sur la sévérité de l’insomnie et l’anxiété situationnelle des femmes âgées de 40 à 60 ans provenant de la région de Québec. Quarante-sept femmes de cette cohorte d’âge, ayant des symptômes d’insomnie, ont été réparties aléatoirement dans le groupe expérimental (n = 25) recevant quatre séances en polarité ou dans le groupe contrôle (n = 22), recevant de l’information sur les saines habitudes de sommeil. Les mesures ont été recueillies pré, post-intervention et deux semaines post-intervention à l’aide des questionnaires intitulés l’Index de sévérité de l’insomnie (Morin, 1993) et l’Inventaire d’anxiété situationnelle (Forme Y-1) (Spielberger, 1983). À la suite des interventions qui se sont échelonnées sur huit mois, nous notons une différence significative entre les groupes et entre les temps de mesure en ce qui a trait aux scores d’insomnie (F = 28,66, p < ,0001) et d’anxiété (F = 14,14, p < ,0001). Les deux hypothèses ont été confirmées.
Les femmes âgées de 40 à 60 ans, qui ont reçu l’intervention en polarité, présentent une diminution significative de la sévérité de leur insomnie ainsi qu’une diminution significative de leur niveau d’anxiété situationnelle comparativement à celles du groupe contrôle en post-intervention. Les recommandations visent le développement d’intervention pour la gestion des symptômes d’insomnie et d’anxiété. Cette étude incitera les infirmières concernées par la promotion de la santé et le bien-être à bonifier leurs interventions infirmières en vue de permettre aux femmes en transition ménopausique de satisfaire leur besoin vital de dormir. -- Mot(s) clé(s) en français : polarité, intervention, approches complémentaires de soins, femmes d’âge moyen, insomnie, anxiété. -- ABSTRACT: Due to insomnia, 4 out of 10 women aged from 40 to 60 years old are deprived of sleep, a necessity of life (Blümel and al., 2012). Strongly associated with insomnia, anxiety tends to increase throughout the menopausal transition (Terauchi and al., 2010). Insomnia and anxiety can bring adverse effects on physical and mental health, yet only 20% of people who complain of insomnia are properly taken care of (Ohayon 2007). The intervention is a pretest-posttest experimental design with a control and an experimental group in order to assess the effectiveness of an intervention in polarity on insomnia and anxiety of women aged 40 to 60 years old living in the Quebec City region. Forty-seven women in this cohort with symptoms of insomnia were randomly assigned to the experiment group (n = 25) receiving 4 sessions on polarity or to the control group (n = 22) receiving information on healthy sleep habits. Data was collected during the pretest, the posttest and two weeks after the intervention using the Insomnia Severity Index (Morin, 1993) and the State-Trait Anxiety Inventory (FormY) (Spielberger, 1983). Following the interventions that took place over eight months, results show a significant difference between the groups and the measurement time with respect to insomnia scores (F = 28.66, p <.0001) and anxiety scores (F = 14.14, p <.0001).
The two hypotheses of the study were confirmed, women aged 40 to 60 years who received the intervention in polarity show a significant decrease in the severity of their insomnia and of their state of anxiety compared to those of the control group during post intervention. The recommendations are intended to enable the development of intervention to reduce the symptoms of insomnia and anxiety. This study will encourage nurses, who are concerned with health promotion and well-being, to improve their nursing intervention in promoting healthy sleep of women in their menopausal transition. -- Mot(s) clé(s) en anglais : polarity, intervention, complementary treatment methods, middle-aged women, insomnia, anxiety.
